Applicants would need to provide verification of training for their scope of privileges being requested. 
If this included "on-the-job training", a letter obtained from the physician/facility that provided the training would need to be obtained. 
Our Credentials Committee normally does not accept copies of letters submitted by the appicant addressed "To Whom it May Concern"
